A Third Party Authorisation form is used in the circumstances that you wish to be authorised to act on behalf of another person (usually the account holder) or if you wish to authorise another person to act on your behalf, to deal with ComReg’s Consumer Care team about a complaint relating to a telecommunications service provider.
To request an application form please submit your contact details and then select the relevant Third Party Authorisation application form type you need from the drop down list when prompted.
A member of our team will subsequently email you the relevant form and instructions on how to submit the completed form and relevant supporting documentation/valid ID.
Please check your email spam folder if you have not received the application form requested within 3 working days of your request and consider adding ‘firstname.lastname@example.org’ to your Safe Senders list.
